What it does
The hollow between the lower lid and the top of the cheek is called the tear trough. When it deepens it casts a shadow, and the shadow is what reads as tiredness. That is why sleep does not fix it and why people keep asking whether you are alright.
Filler placed deep in that hollow, down on the bone, lifts the floor of it slightly so light stops falling into a trench. The change people see is a shadow softening rather than anything being added to your face.
It is a small amount of product. Usually well under a millilitre in total across both sides, placed at two or three points through a cannula. This is the area where less is genuinely more, and it is the area where clinics get greedy.
Not everybody has a hollow. Some men have pigment in the skin, some hold fluid, some have loose skin, and filler is the wrong tool for all three.

Consultation, 15 minutes
This one takes longer than the others, because the assessment decides whether we treat you at all. A doctor looks at the hollow, the skin, the fluid and how the area moves. A meaningful share of the men who come in for this are better off with something else, and they get told that.

Numbing, 10 minutes
Cream first. The area is more sensitive than the jaw and it gets longer.

Treatment, 15 minutes
A cannula through one entry point on each side, product placed on the bone, small volumes, checked with you sitting up.

Two weeks later
Free review. The under-eye holds fluid more than anywhere else on the face, so what you see on day two is not the result. Two weeks is.

Will I look done?
Under-eye is the area where doing too much shows fastest, so this is the treatment we are most conservative with. Filled correctly, the hollow under the eye is softened and the shadow lifts. Overfilled, it puffs, and everyone can see it.
Not everyone is suitable. If the darkness under your eye is pigment rather than shadow, or if there is a lot of fluid there, filler will not fix it and may make it worse. A doctor will tell you that at the consultation and you will not be charged for finding out.

Will this work for me?
It depends what is causing the darkness. If it is a hollow casting a shadow, filler usually helps a lot. If it is pigment in the skin, or fluid, filler will not fix it and can make it look worse. A doctor will tell you which one you have got, and you are not charged for finding out.
Will people be able to tell?

Does it hurt?
It is uncomfortable rather than painful. We use numbing cream first, and the filler itself contains a local anaesthetic. Anti-wrinkle injections feel like a scratch and are over in a couple of minutes. Most men tell us it was less than they expected.
How long does it last?
Usually 12 to 18 months, depending on the area, the product and how quickly you metabolise it. Men often metabolise filler slightly faster than women, which is worth knowing before you compare notes with anyone.
Is it reversible?
Yes. We use hyaluronic acid fillers, which can be dissolved with an enzyme. That is one of the main reasons we use them. Anti-wrinkle treatment is not dissolvable but it wears off on its own in three to four months.
How long does the swelling last?
Usually a few days. The under-eye area holds fluid more than anywhere else on the face, so it is normal to look slightly puffy at first and to see the real result at two weeks. That is exactly why the review appointment exists.
What if my dark circles are genetic?
Pigment and hollowing are two different problems and a lot of men have both. Filler addresses the hollow. Pigment is a skin problem and needs a different approach. You will get an honest answer at the consultation about which is which.
